EWS Radio Receiver Activation Page

FOR RESIDENTS OF WOODWARD, HARPER AND ELLIS COUNTIES ONLY.  IF YOU LIVE IN
ANOTHER COUNTY PLEASE CONTACT YOUR COUNTY EMERGENCY MANAGEMENT DIRECTOR.

To make sure that your radio is receiving properly, a test message will be sent each Wednesday at noon
and again at 7:00 p.m.  Your radio's alarm will not be activated during this test.
Instructions:

Complete the form on the right.  Use
the "Tab" button on your computer to
go to the next question, or click on
the appropriate box with your mouse.  
Click on the "Activate My Radio"
button at the bottom when finished, or
"Clear Form and Start Over" if you
need to erase the form and start
over. ONLY CLICK THE "ACTIVATE"
BUTTON ONCE. If you have a slow
internet connection, it may take a few
moments to send the information.
Your computer will be re-directed to a
confirmation page once your
information has been submitted.
Please allow 3 full business days for
your information to be processed. If
you ever need to change your
information, please return to this web
page and correct any information.

Information provided will be treated as
confidential and will not be shared
with anyone other than emergency
management personnel in your
community.  The information will only
be used to set up your radio.

Government entities using this system
are not responsible for radios which
are lost, stolen or damaged, or for
improperly installed radios which do
not receive a warning signal.

an external antenna to receive the warning signal.]

we need to alert you to special information in your
neighborhood, such as alerting you to a chemical spill
near your home or business, a wildfire moving towards
you, a prison escapee in your area, etc.  If you live in a

nearest town. For example, if you live at Rt 1 Box
1, give us your home/business's location from the
nearest intersection, such as 1/2 mile west of
county road NS100 and EW200.

a text message) in addition to the alerts to your radio,

are an AT&T/Cellular One customer in NW Oklahoma,
your phone's address is the 10-digit phone
number@mobile.celloneusa.com.  If you are a Pioneer
Cellular customer, your phone's address is the 10-digit
phone number@zsend.com.  If you are not sure what
your cell phone's email address is, dial 611 on your cell
phone and ask what the address is.

to leave your home, if you had to evacuate? (Please
answer "yes" if you are wheelchair-bound, or have any
other physical handicap which would not allow you to
leave your home on your own,
and if you don't have
anyone living with you who could assist you in leaving

12.  Your receiver can be sent "informational" alerts
on major hurricanes, floods, earthquakes and volcanic
eruptions.  Notifications will only be sent for major
worldwide events, and your radio will only sound two
low rapid beeps when these alerts are received.

IMPORTANT NOTE - PLEASE READ

If you have children who attend a school in Ellis, Harper or Woodward County please contact your
school's principal to see if your school participates in the school messaging system and asked to be
signed up for school alerts.  The principal will forward your information to your county's emergency
manager who will enter your information into the school alert system.  If you do this, please make sure
that when your child changes schools or graduates, you notify the school to take you out of their
system so that you won't receive unnecessary alerts from the school.

If you live in another county in NW Oklahoma and are not sure if they have a school alert system,
please contact your county's emergency management director and ask if they participate in the EWS
system.

If you are a volunteer or paid member of a public safety entity (Firefighter, Law Enforcement Officer,
EMT, etc.) or Red Cross Volunteer in Ellis, Harper or Woodward Counties please notify your Chief or
Supervisor that you would like to be added to the alert system.

If you would like to purchase a radio for a school, hospital, nursing home, etc. in Ellis, Harper or
Woodward Counties please contact your county's emergency management office and they will get
information to you on where these radios are needed.  Contact numbers and e-mail addresses are
listed to the right.
